Notification of Positive COVID-19 Test

If you have returned a positive COVID-19 test result, please complete the form below to notify the Norwood Basketball Club.

Please be aware that your name and/or playing number will be shared with fellow team/squad mates and opposition players to allow them to make an assessment as to whether they (or their child) meet the close contact personal interaction conditions as set by SA Health.

The health, safety and wellbeing of our basketball community remains a priority and it’s important that we carefully follow the restrictions in place by the State Government.
